Fill out this information for each child that will be in attendance on your first visit. Upon arrival you'll only need to enter the last 4 digits of your phone number to check those children in. It may ask you to use a separate email address for each child. (We're working to correct this, and we can change it on your first visit.) If you have any issues we can help you upon your arrival.
